In order for a student-athlete to participate in Bluejay Athletics, the following 4 forms must be completed, signed, & returned to the NJSP Athletic Department before the first official practice in a respective sport:
A student-athlete will not be allowed to participate in any team practice or sporting event until these 4 forms are submitted to the NJSP Athletic Department.
