Athletic Forms

In order for a student-athlete to participate in Bluejay Athletics, the following four forms, linked below, must be completed, signed, and returned to the NJ-SP Athletic Department before the first official practice in a respective sport.

A student-athlete will NOT be allowed to participate in any team practice or sporting event until these four forms are submitted to the NJ-SP Athletic Department.
